My name is Cristian Gabriel and I was born on June 5, 2018.
I am a cute little boy, diagnosed with spastic tetraparesis (a form of brain paralysis causing partial or total loss of limb and torso use).
I was born premature, at 29 weeks, by C-section. Because of some complications, physicians had to perform an emergency C-section, not before asking us: “THE MOTHER or THE CHILD?” I think that was the most terrifying question my parents ever heard. Both I and my mother survived, by a miracle.
At birth I had only 1300 grams, and the physicians held me in the hospital for two months. I thought that my struggle was over and that everything would be fine. But it was not even close to this, because at nine months I could not hold my head and I could not sit. Therefore, I got back to hospital with my heart tight, where I received the diagnostic of spastic tetraparesis and neuromotor development delay.
In December, I was subject to a surgical intervention on my little legs, but the physicians told me it is only a first surgical intervention from a long range of therapies for recovery, and I need tens of therapy hours to see progresses and improvements.
Upon the recommendation of the kinesitherapist, I should be also subject to a surgical intervention of miofibrotomy, intervention that can be performed at a clinic from Chișinău.
I need a lot daily sessions of kinetoterapie, logopedics, occupational and sensorial therapy, and because for therapy sessions I have to go to clinics from different cities, transport and accommodation expenditures are added to the therapy costs.

My therapies cover all possible forms: swimming, kinesitherapy, sensorial integration therapy, occupational therapy, various therapeutic methods and complementary techniques (therapeutic massage, stretching, elongations, and thermotherapy).

The forms can be filled-in and submitted at ANAF until May 25 of each year, to redirect the withheld tax in the previous year.
*According to the new Tax Code, directing the amount representing up to 3.5% of the tax on the income earned can be made to the same beneficiary for a period of 2 years (checkmark in section II, point 2) and it can be renewed after the expiry of the said period)

The form can be submitted also by ANAF online service – Virtual Private Space, if you have electronic signature.
